YSHA Occupancy Department will need copies of: Social Security Cards and Tribal Enrollment, and Income Verifications for all household members. (Including Children)

Applicant: Remember if you do mont submit the required documents with your application, it will be incomplete until our department receives them from you. Falsifying information will disqualify your application and housing with the YSHA.

1. Copies of all requested personal documents. (Social Security Cards/Tribal Enrollment)
2. One (1) Landlord Reference
3. One (1) Credit Reference
4. Three (3) Personal References
5. Income Verification
6. Child Care close receipts

Your application will be held for up to 12 months or until you have been selected for a home. A year after your application, you will be requested to submit and verify by your signature a new application. A notice will be sent to you at the address soon your application. To assure that your place is kept on the waiting list, it is your responsibility to make sure that your application is received and completed all documentation is received by the YSHA Occupancy office. Please be advised that background checks will be done before new move-ins are assigned to a unit and signing of the lease.
